What is the service?

Te Ara Tiaki Pakeke is the Tairāwhiti healthy ageing service here to support older adults to age well and live well in the home of their choosing.

 

Services offered

Home and Community Support Services – HCSS

For short term support or following significant change in health circumstances the Home and Community Support team can provide home help and or personal cares based on need.

 

Needs Assessments and Services Coordination – NASC

The Ministry funds services for people with a physical, intellectual and/or sensory impairment or disability that is; likely to continue for a minimum of 6 months and or reduce their ability to function independently, to the extent that ongoing support is required.

 

Health of older person – HOP

Is aged 65 years and older, or as young as 55 years with an age related condition.

 

Palliative

Supporting those during the end stage of a terminal medical condition.

 

Long-term supports & chronic health conditions - LST & CHC

Supporting patients younger than 65 years who are living with a long term or chronic health condition that is not a disability.

 

Mental Health Needs Assessments and Services Coordination

Clients that access this service are referred by a clinician specialist from secondary mental health service or from Te Whare Awhiora.

 

Child and adolescent mental health service – Te Whare o Te Rito (iCAMHS) respite

Clients accessing this service are referred by clinician specialist.

 

Fracture Liaison Service

A service which screens patients 50+yr who fall and break a bone that is not in the hands, feet of skull. There patients are supported through the Osteoporosis New Zealand pathway.

 

Falls Prevention

Education and advice to individuals, family and community on preventing falls.

 

Clinical Nurse Specialist - Geriatric Specialist

Our Clinical Nurse Specialist works alongside the Geriatrician and the wider team with specialist Comprehensive Geriatric Assessments, and Stroke survivors.

 

Allied Health Response Team – AHRT

This is a team response service, providing assessment of the frail older person to ensure safety, and to facilitate independence at home following a presentation to the emergency department or a serious decline.

 

Social Work

Providing advocacy, navigation, advice, support and or coordination regarding; Health Services, Social and Financial concerns, Enduring Power of Attorney (EPOA) / Protection of Personal and Property Rights Act (PPPR) affecting respite and care options.
